You are invited to participate in a survey on physical activity in people with limb loss, limb difference, or limb impairment. In this survey, you will be asked questions about your participation in physical activity and your experiences with using assistive devices for physical activity. It will take about 10 minutes to complete the survey.
You are eligible to participate in this survey if:
- You have limb loss, limb difference, or limb impairment OR you are a parent responding on behalf of your child with limb loss, limb difference, or limb impairment
- You live in the United States or a United States territory
If you complete the survey, you may enter a raffle to win a $10 Amazon gift card. Instructions will be provided at the end of the survey.
Your participation in this study is completely voluntary. There are no foreseeable risks associated with this survey. Your survey responses will be strictly confidential and data from this survey will be reported only in the aggregate. However, if you feel uncomfortable answering any questions, you can stop at any point.
